USS Connecticut: Navy investigation reveals that submarine crashes in the South China Sea are “preventable”

The nautical review team, including the commander, was unable to find and mark at least 10 underwater hazards near the location of the ground, and investigations were found, and the team erroneously concluded that the submarine would operate in an open area. Submarines also suffered from “low standards” because the ship’s leadership did not hold seafarers responsible for navigation errors and defects.

The investigation also revealed a previous accident at USS Connecticut. In April 2021, the submarine collided with the pier at Point Loma, a naval base in San Diego. At that time, investigations revealed that nautical, planning, and maneuvering standards had declined, but the accident was canceled as “abnormal performance, not system failure.”
